Death in Acute Hospitals

This will review the care of patients who die in acute hospitals. This report is a based on the 2002 report 'Functioning as a Team' and we will be concentrating on the effects of teamwork on the care of patients. Since the publication of that report our remit has been extended to include medical patients as well as surgical patients, so the new study will also include all specialties rather than just surgery.

A spreadsheet was sent out to all acute sites on 11th July 2007 asking for a submission of all death data from 1st October 2006 to 31st March 2007. The deadline for this information was 3rd September 2007. Questionnaires for deaths included in the study along with covering letters will be sent out shortly for completion.

We are also using this dataset to identify patients for the Acute Kidney Injury study in order to minimise the number of cases that the local reporter will have to return to NCEPOD.

The protocol for this study has been approved by the Expert Group and NCEPOD Steering Group and is available to download here.

We are now in the process of sending out the Medical Surgical questionnaires to Local Reporters and to Clinicans, which should arrive during April 2008.

There are three questionnaires related to this study:

  • Organisation questionnaire – to gain aspects of organisational care at your site that is related to this study. This will be sent out towards July 2008.

  • Medical Surgical questionnaire – we will only be sending 1 questionnaire for each clinician to gain information about the first death under their care in this time period. To download and print this questionnaire please click here.

  • Anesthetic questionnaire – For cases where an anesthetist was involved, a questionnaire will be sent out to them. These will be sent out from June 2008 onwards. To download and print this questionnaire please click here.

I was not the clinician responsible for this patient, what should I do with the questionnaire sent to me?
Either pass on the questionnaire to the correct clinician and let NCEPOD know the new name. Or, if you don't know who the correct clinician was please return the questionnaire to NCEPOD stating the reason why it is blank.
Do I have to complete this questionnaire?
Yes, all questionnaires should be completed as mandated by the GMC in Good Medical Practice (P14).
My deadline has passed to return the questionnaire, what shall I do?
If you do not return your questionnaire you will receive a reminder which will extend your deadline,. However, please do contact NCEPOD and we will give you a new deadline. All data returned at the moment will be included in the study.
